services for intake and enrollment or referral to community agencies.

Populations Served by the Admissions and Referral unit
This unit provides intake, enrollment and/or referral for adults, families and children of any age
who are seeking services.
Hours of Operation

Behavioral Health Services
- Behavioral Health services are recommended following an intake by a Triage Specialist.
This unit provides assessment and service recommendation.

who are seeking services.
Hours of Operation
- The Guidance Center’s administrative offices are open 8:00am to 5:00pm, Monday
through Friday, excluding national holidays. - The administrative offices are located at 2187 N. Vickey St., Flagstaff, AZ, 86004.
- The Admissions Department is also located at 2187 N. Vickey St, Flagstaff, AZ 86004.
- Appointments are available Monday through Thursday from 8:00am to 5:00pm, and
Friday, 8:00am to 12:00pm. - Evening appointments are available at 5:30pm Monday through Thursday.
