Outpatient Services
Community Living Services provides services to adults age 18 and over, diagnosed with a serious
mental illness (SMI) and to adults having other mental health diagnoses that are funded through
Title 19.  Services offered at CLS include:  Individual/Group and Family Counseling (Office and
Out of Office); Health Promotion; Living Skills Training; Case Management; Screening; Core
Assessments and Addenda; Evaluation and Diagnosis; Title 36 Court Ordered Services
(Prepetition Mental Health Screening; Mental Health Evaluation; Mental Health Services); WRAP
Plan Development; Personal Assistance, and Peer Support. If needed services are not currently
available at The Guidance Center client will be referred to a licensed provider in the community.
CLS staff includes paraprofessionals, BHT’s and BHP’s.

  • CLS hours are 8:00am to 5:00pm Monday through Friday. Group services are offered three
    days weekly for a total of six hours.
  • Individual counseling and case management are available 8:00 am to 5:00pm Monday
    through Friday. Each week clinicians provide 340 to 425 hours of clinical services.

The overall goal of Community Living is to improve the behavioral health of persons with serious
mental illnesses or other mental health problems through the provision of high-quality, cost-
effective outpatient services.

Case management services are provided in office and out of office, as a supportive
services, in order to enhance treatment goals and effectiveness.


Case management activities include:
  • Assistance in maintaining, monitoring and modifying covered services;
  • Brief telephone contact or face-to-face interactions with the consumer, his/her family or
    other involved party for the purpose of maintaining, or enhancing a consumer’s function;
  • Assistance in finding necessary resources;
  • Communication and coordination of care;
  • Outreach and follow-up of crisis contacts and missed appointments;
  • Staffings;
  • Other activities as needed.

Independent living for the SMI population is offered at Ponderosa Plaza and Lewis Lodge. Case
managers provide skills training and support necessary for clients to live independently in the
community.
