STOHP (Street to Home Partnership) Program Care Coordinator
Full Time
Required:
• Bachelor’s degree in behavioral health field plus two years related experience required.
• Clear driving record and ability to obtain AZ Fingerprint Clearance card required.
• Flexibility and motivation are a must.
• Strong competency with crisis management, needs assessment, organization, communication and data
reporting skills necessary.
• Skilled in independent judgment and problem solving with respect for all partners and identified consumers
in the program.
The primary focus of this position is to coordinate care for adults that are accepted into the StoHP program.
This position is responsible for representing TGC with stakeholders, delegating program needs with community
partners, managing access to care, implementation of evidence-based care models such as WRAP and Housing
First.
