Youth And Transition Case Manager
Full Time
The primary function of this position is to provide case management, support services and rehabilitation services
to enrolled members between the ages of 18-21 with a maximum caseload of 30. Preference will be given to
members who are currently in out-of-home care, have other state agency involvement, are homeless, and/or are
former CMDP clients. This position will be expected comply with Clinical Accountability Guidelines. Services will
be community based, delivered at times and days that meet member needs and preferences, and according to
evidence-based practices. Service planning and coordination shall be consistent with ADHS/DBHS Transition to
Adulthood Practice Protocol.
Required:
• Master’s Degree or Bachelor’s Degree in a field related to behavioral health
• Associates Degree and at least two years of behavioral health experience
• High School Diploma and 4 years experience in the behavioral health field -or-
• High School Diploma and 2 years experience in the behavioral health field and 18 credit hours in
• Sociology, Social Work, or Psychology or a closely related field completed in the previous 4 years.
• Current driver’s license and able to pass MVR screening.
• Must be 21 years of age
• Ability to obtain CPR and 1st aide certification
• Ability to complete crisis intervention training
• Ability to work flexible hours
• Ability to travel to other clinic sites if needed
• Ability to work as part of a team to coordinate client care
Preferred:
• One (1) year of case management experience.
• Prior experience with the Adolescent Abuse Population.
• Experience providing crisis services
• Knowledge of community resources available, including benefits and entitlement process.
• Understanding of managed care.
• Trained in crisis intervention.
• Familiar with SA and co-occurring diagnoses, treatment and special needs.
• Familiarity with commonly used psychotropic drugs and their side effects
